Inspector notes: Observed a pressurized Y-Valve installed downstream from the mop sink AVB. Establishment has a direct water connection for the chemical dispenser that is currently in disrepair and not in use. PIC removed shut-off downstream from atmospheric vacuum breaker at time of inspection. Shut-offs, including chemical dispensers and sprayer nozzles, are not approved for continuous installation downstream from atmospheric vacuum breakers. Advised PIC to repair the direct water connection for the mop sink ASAP. PIC stated they will disconnect the hoses immediately after use until repairs are completed

Improper cooling methods used (corrected on site)
Inspector notes: Observed tightly wrapped packages of roast beef separated by parchment papers measuring 50-52*F interally. Per PIC, the roast beef was sliced between 11:30-11:45am and placed into the prep cooler. In the other prep cooler, observed halves iceberg lettuce at 48*F and thinly shaved lettuce measuring 50-55*F, internally. Observed 6-8" of cooling lettuce in the pans. PIC stated the lettuce was washed and sliced less than 2 hours prior. TCS foods must be cooled in uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. All TCS foods being cooled in deep containers were transferred to shallow containers to allow for rapid cooling. Discussed that prep coolers are designed to hold already cooled TCS foods and are not intended to cool TCS foods. Advised PIC to utilize the walk in cooler or freezer to rapidly cool TCS foods
