Inspector notes: Observed buckets on the ground in back storage area and on the counter near the microwave. After discussion with the person in charge the ceiling has begun to leak after a recent storm. No overhead contamination of food or food contact surfaces observed. Observed cove base on the wall under the three compartment sink next to the mop sink missing. Physical facility must be maintained in good repair at all times. Repair physical facility prior to next routine inspection

Inspector notes: Observed two filled spray bottles in the chemical storage area with no labels. Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material. After discussion with the person in charge it was determined one of the spray bottles, filled with red liquid, was a general cleaner. The person in charge labeled the bottle at time of inspection. After discussion with the person in charge it could not be determined what chemical was filled in the other spray bottle (cloudy/colorless liquid). The unknown chemical spray bottle was discarded at time of inspection
