Two units storing these items were in disrepair and items were placed in ineffective ice baths. Cups of coleslaw were also stacked too high. All time/temperature control for safety (TCS) foods being cold held need to be at 41°F or below. COS by adding ice directly into container storing coleslaw. Lettuce was discarded.
