Inspector notes: Upon arrival, observed no hand soap or paper towels available at the hand washing sink located in the main kitchen. No hand soap, paper towels, or running water available at front bar area either. Hand sink in kitchen blocked by garbage cans. Repeat violation.

Inspector notes: Observed dish machine's chlorine concentration at 0ppm. Advised PIC to not use dish machine until it is repaired. Advised PIC to use 3 compartment sink to wash dishes until dish machine is repaired. Repeat violation.
