For example, fish, crustacean shellfish, wheat (flour), and milk, are items served at this facility, four of the nine major food allergens. Consumers must be notified by any method (physical /electronic signage) that there are menu items that may contain a major food allergen(s) and may be used as an ingredient(s) in unpackaged food items prepared at this facility. Corrective Action: Notification - physical/electronic signage should be placed anywhere customers place or pick up a food order to disclose allergen information, for example, near the front entry/waiting area, on dining tables, at the bar, and customer counters.
