Inspector notes: The presence of rodents is not controlled as evidenced by an over accumulation of rodent droppings on the floors in the kitchen, under the donut case, and under the cash register. The presence of insects, rodents, and other pests must be controlled by routinely inspecting the premises for evidence of pests, using acceptable methods to control pests, eliminating harborage conditions, and routinely inspecting incoming shipments of food and supplies. - A full evaluation by pest control must be performed before the next inspection. The full evaluation must include frequency of treatment, types of bait, traps, initial kill numbers, structural findings, and technician assessment. Establishment must fulfill all of the recommendations by pest control. Pest control to increase frequency until rodent activity is resolved. Full evaluation to be completed before next inspection. - Establishment to create a plan of action to follow alongside the findings from the evaluation. Plan of action to include cleaning and sanitizing of all affected areas with rodent droppings. Establishment to ensure pest sighting/activity logs are completed when activity occurs. Disposing of any adulterated product daily. Sealing openings that may allow for pest entry. Plan of action to be completed before next visit.

Inspector notes: A rodent carcass was observed on a glue trap in the kitchen. Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the PREMISES at a frequency that prevents their accumulation, decomposition, or the attraction of pests.

Inspector notes: The floors in-between the refrigeration units and under the prep table in the kitchen area has an accumulation of soil residue and food debris. The physical facilities shall be cleaned as often as necessary to keep them clean.

Inspector notes: The ceiling in the front prep area is covered with tiles that are not smooth, durable, and easily cleanable.
