Inspector notes: -- Under 3-compartment sink and near dishwashing area, observed nymph and adult German cockroaches. The person in charge (PIC) provided the most recent pest control report from 1/15/25 showing floors 2 and 3 of facility were treated for cockroaches. The establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Advised the PIC the establishment must have kitchen treated by a certified/licensed pest control specialist to eliminate current pest control problem and a re-inspection will be completed within 10 days

Inspector notes: -- Observed floor drain for dishwasher with water backing up on floors and the drain unable to clear from the point of discharge. An employee attempted multiple times to clear the debris and unclog the drain, but was unsuccessful. The inspector ran additional plumbing fixtures throughout the facility including: 3-compartment sink, prep sinks, hand sinks, and the mop sink for several minutes to verify all other drains remained cleared. The dishwasher will be turned off and unused until drain is repaired. The 3-compartment sink was set up at the time of inspection for washing and sanitizing dishes. Sewage wastewater shall be properly drained to a system in accordance to LAW
