Inspector notes: *SECOND CONSECUTIVE VIOLATION* Employee in the cookline area cut open multiple packages of raw beef items, then went to cut cooked waffles, avocados, and bananas on the same cutting board in the cookline area. There was also multiple bananas stored on the cutting board directly next to the raw beef knife. The PIC removed the equipment to be properly cleaned. Ensure that equipment is properly cleaned each time there is a change from working with raw FOODS to working with READY-TO-EAT FOODS and any time contamination may have occurred. After discussion with the person in charge, it has been determined that the root cause of the violation was training staff and a verification breakdown which resulted in the violation
