Inspector notes: Observed deep container of vegetable soup in the reach-in in the back of the restaurant. Per PIC the soup was made at home and transported to the facility for sale. Discussed with PIC all food must be obtained from an approved source as defined by law. Food prepared in a private home may not be used or offered for human consumption in a food establishment. All food from unapproved source was discarded at time of inspection

Inspector notes: Observed in the back of the facility boxes of single use items and coffee equipment that are stored direction on the ground. Discussed with PIC equipment, linens, single use items and food must be stored at least 6 inches off the ground at all times to prevent contamination from the premises

Inspector notes: Observed a small ice machine in the back of the restaurant that has a condensation line dripping into a bucket. Discussed with PIC the condensation line shall drain into a floor sink. Per PIC the machine will be relocated to the front by the hand wash sink
