Inspector notes: Observed significant accumulation of organic matter along the interior of the ice machine. Per person in charge, the ice from the ice machine is used in customer's smoothies. The establishment shall wash, rinse, and sanitize the interior of the ice machine at a frequency to prevent the accumulation of organic matter and other debris

Plumbing system not maintained in good repair

Inspector notes: Observed multiple deficiencies located at the mop sink. Observed chemical dispenser attached directly to the mop sink faucet via a splitter y-valve. At the time of inspection both ends of the y-valve were in the off position and both the hot and cold water knobs were turned to the on position at the faucet creating constant pressure. The atmospheric vacuum breaker (AVB) backflow prevention device was observed to have a loose fitting cover. The AVB device appeared to be functioning properly still. The establishment shall provide a separate, dedicated water line for the chemical dispenser and shall remove the y-valve from the mop sink faucet
