Inspector notes: -- Chlorine sanitizer undetectable on dishes ran through low temp sanitizing dishwasher. The dishwasher was primed and reran multiple times and it consistently tested below the 50ppm-100ppm requirement. The PIC stated a work order will be placed to repair dishwasher. PIC relocated all food-contact surfaces to ware washing area to be washed, rinsed, and sanitized at the time of inspection. Of note: A technician arrived onsite during the inspection and repaired the dishwasher. Inspector verified concentration ranged between 50ppm-100ppm at the time of inspection

Inspector notes: -- Multiple containers of turkey and ham sandwiches in the doctor's lounge reach-in without date mark labels on them. The PIC stated the sandwiches were prepped on 10/10/25. All RTE/TCS foods were properly date marked at the time of inspection. RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1)

Improper cooling methods used (corrected on site)
Inspector notes: -- In walk-in freezer, observed several tightly covered hotel pan size containers approximately 6" deep of sliced turkey and buttered corn with internal temperatures ranging from 47°F-53°F using probing thermometer. Per PIC, the items were temporarily removed from the walk-in refrigerator about 1 hour prior to the inspection while employees were putting away delivery. Reviewed approved cooling methods with the PIC: TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. All TCS foods cooling were uncovered to allow for rapid cooling. The inspector verified the TCS items were rapidly cooled to 41°F at the time of inspection

Inspector notes: -- Observed tomato soup, brown gravy, and mashed potatoes on patient line with internal temperatures ranging from 114°F-129°F. The PIC stated the items were reheated and placed in hot holding unit approximately 1 hour prior to the inspection (10:30 AM).
