The document was provided with signatures dated for 4-23-26 instead of prior to the inspection. CA: Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food; Pf COS- Team members onsite signed the reporting agreement during the inspection. Lead instructed to ensure that all employees are trained and sign the document by their next shift.
