Inspector notes: OBSERVATION: Employee health reporting agreement is used as the proof for training/acknowledgement. The document was provided with signatures dated for 4-23-26 instead of prior to the inspection. CA: Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food; Pf COS- Team members onsite signed the reporting agreement during the inspection. Lead instructed to ensure that all employees are trained and sign the document by their next shift.

Inspector notes: OBSERVATION: No written vomit/fecal clean-up procedures provided during the inspection. PIC unsure of the mixture nor contact time for the alternative EPA registered chemical that was onsite. PIC informed to email a photo of the written instructions for the EPA registered chemical effective against Norovirus to include the mixture and contact time by 4-30-26 nakia.robinson@dph.ga.gov

Inspector notes: Observed food handlers while preparing sandwiches wearing watches and bracelets. CA: (g) Jewelry. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food.

Inspector notes: Observed employee while preparing sandwiches with hair longer than a 1/2 inch without a hair restraint. CA: 1. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. COS: employee discontinued preparing sandwiches.
