Inspector notes: The facility no longer has record of the signed conditional employee agreements. CA: PIC could not provide documentation that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food. Director of Operations reported that the printed and signed copies were stored in a binder that is no longer in the office area. Record not digital. Director of Operations will get employees to resign agreements and retain these copies on file.

Inspector notes: Observed food and physical debris on nonfood contact surfaces throughout the main kitchen including the hand sink edges and faucet, equipment gaskets, the rails under the lids and bottom shelves beneath the preptop cooler, and dry item storage shelves by the pizza oven. CA: These surfaces must be cleaned and sanitized as often as necessary to prevent debris accumulation in order to prevent contamination of food items, in use utensils, single use items, and employees' hands.
