Inspector notes: Upon arrival to facility, no person in charge (PIC) was present. There must be a person in charge on the premises of the food service establishment at all times. The person in charge shall ensure compliance with the following: In-use utensils: properly stored; Garbage/refuse properly disposed; facilities maintained; Physical facilities installed, maintained, and clean; Adequate ventilation and lighting; designated areas used. Corrective Actions: Employee called PIC and they arrived before the inspection was concluded. All items mentioned were brought to the PIC's attention.

Inspector notes: *First Consecutive Repeat Violation** Observed an in-use utensil (knives) stored in crevices between equipment in between uses. During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored: On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized at a frequency specified under DPH Rule 511-6-1-.05(7)(b) and (8)(a). Corrective Actions: PIC removed knives from crevices and placed them in the 3-compartment sink to be washed, rinsed, and sanitized before reuse.

Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur. Corrective Actions: PIC moved tumbler to designated area.
