Inspector notes: Observed no employee health policy in the facility. CA: The permit holder shall require food employees and conditional employees to report to the CFSM and person in charge, information about their health and activities as they relate to diseases that are transmissible through food. A food employee or conditional employee shall report the information in a manner that allows the CFSM and person in charge to reduce the risk of foodborne disease transmission, including providing necessary additional information, such as the date of onset of symptoms and an illness, or of a diagnosis without symptoms, if the food employee or conditional employee. PIC will have employees to sign EHP ASAP and keep them on file at facility.

Inspector notes: Observed no thermometers in the facility. PIC stated that they just open the chicken to see if it is cooked. CA: Food temperature measuring devices shall be provided and readily accessible for use in ensuring attainment and maintenance of food temperatures as specified under DPH Rule 511-6-1-.04. PIC will have thermometers in facility to be able to check temps of food products.

CA: There must be a person in charge on the premises of the food service establishment at all times. COS PIC arrived when called. PIC informed that someone has to be left in charge when he is not at the facility and all employees must know who is in charge.

COS PIC removed scoops and washed them and placed them back in the product properly with the handle sticking up.

Inspector notes: Observed a microwave in the facility that is not commercial grade. CA: Food service equipment must be commercial grade equipment and designed and built according to standards set by American National standards Institute (ANSI) accredited certification programs. Such an accredited program includes, but is not limited to, one offered by the National Sanitation Foundation, or Underwriters Laboratories. Food equipment that is certified or classified for sanitation by an American National Standards Institute (ANSI) accredited certification program is deemed to comply with subsections (1) and (2) of this Rule when used for its intended purpose. PIC will have microwave replaced with a commercial grade one.
