Inspector notes: Observed ice build-up in chest freezer that prevented freezer lid from closing completely. CA: Equipment shall be maintained in a state of repair. COS: No COS observed. Chest freezer must be defrosted and cleaned free of ice build-up so that freezer lid can be closed properly. This will be rechecked by the health department.

Inspector notes: Observed food items (papaya, cottage cheese, and raw shelled eggs) in reach in cooler that PIC stated were for personal use. CA: Areas designated for employees to eat must be located so that equipment and food for customers is protected from contamination. COS: Employee food items were placed in separate area in reach-in cooler away from food for customers. PIC will place foods for personal use in a container labeled "employee only" and store that container at the bottom of reach-in cooler.
