Inspector notes: Observed failure to provide hand soap at the hand sink in the main kitchen by the ice machine. 511-6-1.07(3)(a) - Handwashing Cleanser, Availability (Pf) (3) Numbers and Capacities. (a) Handwashing Cleanser, Availability. Each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap. Inspector notes: Observed food,dust,and dirt build up throughout the facility (floors, wails, equipment and ceiling). 511-6-1.05(7)(d) - Nonfood-Contact Surfaces (C) (d) Nonfood-Contact Surfaces. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Advised PIC of a detailed cleaning.

Inspector notes: Observed broken tiles underneath the 3 compartment sink in the kitchen. Observed ceiling tiles with water stains. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. PIC will have walls and hood cleaned.
