Inspector notes: Person in charge (PIC) not performing duties; not maintaining receiving/corrective action records by employees for key drop deliveries, not having completed employee reporting agreements, not ensure dishes are cleaned and sanitized, not maintaining correct cold holding temperatures, ensuring correct test strips are used for different sanitizing methods, not taking food temperatures correctly. CA: PIC shall be properly trained on food safety protocols for establishment. Corporate trainer(s) shall be contacted to train PIC on protocols. Please inform me of the date, time and location of training. Provide a copy of business card.

Inspector notes: Observed PIC not providing documentation or otherwise satisfactorily demonstrate during the inspection, that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food. CA: Every employee shall have a signed copy of the employee reporting agreement available for viewing either hard copy or electronically. Gave a copy of employee reporting agreement for all employees of establishment including management to complete.

Inspector notes: Observed buildup of slime in the interior of ice machine. CA: Ice machine shall be cleaned and sanitized according to manufacturer recommendation or as often as needed to prevent mold. Contact vendor to clean and sanitize ice machine.

Inspector notes: Observed time/temperature control for safety (TCS) foods (see temperature log) cold held at greater than 41 degrees Fahrenheit in SW prep top cooler and cold hold well in server area. CA: All TCS foods shall be maintained at 41 deg. F. or below. Purchase better containers to hold blue cheese dressing or hold in ice bath. COS: Closed lids on unit and temperatures dropped to correct temperatures. Keep lids closed on prep top coolers when not using.
