Basic - Floor, wall and/or ceiling soiled/has accumulation of debris. Floor under reach in coolers and grill at front counter soiled with food debris. Also, under front counter soiled with food debris. Also, floor of walk-in cooler soiled with food debris. **Repeat Violation**

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Multiple gaskets soiled on coolers.

Basic - Open dumpster lid. On shared dumpster to right of establishment, lid open.

Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Interior of flip top reach in coolers and glass case reach in cooler at front counter soiled with food debris.

Basic - Unwashed fruits/vegetables stored with ready-to-eat food. In walk-in cooler, unwashed tomatoes stored over cut onions and tomatoes.

High Priority - Toxic substance/chemical improperly stored. On shelf above prep table, can of glass cleaner stored over uncovered bread. Employee removed and stored properly. Also, on storage rack near back door, multiple chemical bottles on shelf with bag- in- box soda boxes. **Corrective Action Taken**

Intermediate - Handwash sink used for purposes other than handwashing. In handwash sink at front counter, pan and grill scraper in sink. Employee removed items from sink. **Corrected On-Site**
