Inspector notes: Observed rodent droppings near the card area near the back door. Clean and sanitize to remove droppings. Contact licensed exterminator and provide a detailed IPM report at the next inspection.

Inspector notes: Repair door sweep on the back exterior door. Ensure no daylight.

Inspector notes: Evidence of rodent droppings under the chip display/and behind the small refrigerators in front of the store under the bread shelving at the end of Aisle 5 and in the back room. Move the floor displays remove kick boards sweep cleaning/sanitize the floors and shelvings to remove all new/old rodent droppings. All food on these shelves need to be placed in totes over night. Remove any food items that have been damaged. The back room- everything needs to be pulled away from the wall clean/sweep/sanitize the floors to remove debris urine old/new droppings. All items need to be off the floor at least 6 inches. If you have any food items they need to be put into totes. The exterminator removed one catchat the time of this inspection. The PIC will email over today's extermination report after the inspection. PIC was able to provide IPM reports for 12/31/24 and 1/2/25. Burgess Pest Management John was present today during the inspection. Recommend having an exterminator come out every day until this problem has been corrected. At the next inspection be able to provide a clean extermination report. Pest Reports will be uploaded and attached to the the permit number.

Inspector notes: Observed mice dropping on top of the can goods salsa in aisle Aisle 5. Clean/sanitize all can goods to remove rodent droppings. Observed card board boxes being used to store can goods in Aisle 5. Remove all card board boxes.

Inspector notes: Evidence of rodent droppings under the chip display in front of the store on and under the shelving in Aisle 2 (dog food) Aisle 5 ( chip aisle) under the bread display at the end of Aisle 5 in the walk in room leading into the refrigeration unit on the floor inside the walk in refrigeration unit and on the floor in the back loading deck area. Contact the licensed extermination company and provide us a detailed IPM report. The last extermination was conducted 12/11/24 and 12/02/24. Both reports state no activity. I am seeing a lot of activity. Also the traps are not set properly against the wall under the shelvings. Ensure all traps are set properly to help with the rodent problem. Remove all chips from the shelving inspect the bags to make sure the bags have not been punctured clean/sanitize the shelving to remove rodent droppings. Do the same in the dog food aisle.

Inspector notes: Observed boxes being stored on the floor in the loading deck area. Ensure boxes are being stored minimum of 6 inches off the floor this is going to help with the rodent activity.

Inspector notes: -Observed debis and soil on floors inside the main refrigeration and freezer unit. Clean to remove soil and debis on floor inside both units. Remove any empty boxes. -Observed sticky substance on the shelving in Aisle 4 (Candy Aisle). Clean to remove soil. -Observed debis on the floors throughout the store. Sweep/clean to remove debris.

Inspector notes: Evidence of rodents by the back door on the right side. Found droppings around door and along wall. PIC will need to clean and have pest control come out and do an inspection before re-inspection date. Manager was out sick today. Please email me the last few pest control reports.

Inspector notes: PIC will need to clean the shelves in the walk in under the milk juice and yogurt. All visibly soiled. Retail shelves in the food area (top mostly) have a build up of dust and grime. Please remove.

Inspector notes: PIC will need to clean the floor in the back room to remove old droppings and any debris. Please inform pest control the location you found the droppings. The waslk in floor will also need to be cleaned.
