Inspector notes: Evidence of mice droppings in the back room near the back door on the floor near the break area next to the mop sink on the bottom shelving under the chips on the shelving near the cat litter at the front of the store near the office door. Photos were taken When I pulled back the card shelving I could smell a strong ordor and observed mice droppings. PIC stated that the pest company does not put traps inside the card shelving. This needs to be addressed. Photos were taken. After reading 3 recent pest reports- 10 mice were caught they found a nest under one of the registers. Exterminator states on the reports that cleaning needs to take place every night he needs access to be able to place traps properly traps have been removed at the time of this inspection I pointed out to the PIC that the traps needed to be placed up against the wall I only saw a few traps in the back room. Last report was 5/13/25 5/7/25 4/4/25. CLEAN TO REMOVE ALL DROPPINGS. Contact exterminator to come out and set up more frequent inspections until you receive a clean report.

The door sweeps that were installed prior he recommended another door sweep and the PIC stated that a work order was placed to have all exterior doors repaired. The exterminator also pointed out a few holes that were not visiable due to the boxes and uboats being stored to close to the walls. PIC stated that she put a work order in for these repairs. Recommended having the exterminator come out 2 times a week until there are no catches and then you can speak with the exterminator on what is the best schedule to maintain a clean report.

Inspector notes: At the time of the inspection the 2 bottom shelves in the chip isle did not have any chips stored on them. The PIC stated that they had remove the chips from the shelving 3 days ago and they were considered damaged goods. The kick plates were also removed from this aisle. Evidence of fresh rodent droppings were present on the shelving during the inspection. One of the staff members cleaned up the rodent droppings after the droppings were pointed out to them. The back room also has evidence of rodent droppings near the rear door. Clean to remove rodent droppings. After looking over reports from 1/13/25 12/10/24 11/26/24 10/16/24 9/9/24 8/27/24. On 8/27/24 the exterminator stated in the report that all boxes need to be removed 6-18 inches from against the wall so that traps could be placed and proper cleaning could take place. This recommendation has been on every report since 8.27.25 and no corrective attention as taken place. The exterminator also stated in his report that all the exterior doors need door sweeps. NO daylight should be seen on the bottom of the doors. Also the door leading into the back room I recommend putting doors sweeps on as well. Please look over all these pest reports and ensure that you are taking corrective attention on all the recommendations noted on the pest report. Kick plates have been removed from the chip aisle. Continue keeping this off. Ownership should consider purchasing shelving for the back room. To ensure all boxes are being stored properly off the floor. The exterminator needs to be able to put traps down and the floors need to be cleaned to help prevent with rodent activity. Contact the licensed exterminator and provide a detailed report and an action plan moving forward. Discontinue placing chips on the shelving over night. All chips need to be placed in totes and removed from shelving. Soda shelving and chip shelving need to be cleaned and sanitized to remove soil. Boxes are still being placed on the floor and not on proper shelving. Discontinue using soda racks for storage.

Evidence of rodent droppings on the floor around the mop sink. Evidence of holes in the wall around the mopsink. Clean to remove rodent droppings. Fix hole in the walls around the mop sink. Evidence of rodent dropping on the floor around the break area and around the lockers. Observed candy inside the bottom locker. Clean and Remove candy or any food inside the lockers. Evidence of rodent dropping all throughout the storage/receiving room. On the floor along the walls under the shelving heading towards the break area and around the trash compactor. Clean to remove rodent activy. I asked the PIC if I could see the pest reports which they had in an Eco Lab folder. I went back until Aug 2023 ( I stopped at Aug due to my time) and each report states the same issues. They need to fix the door sweep move boxes off the wall Clean fill in the holes inside and outside the building to stop the rodent activity. Both managers and I walked around the store and we spoke about the importance of cleaning not having food inside the bottom lockers I suggesed asking corporate for kick plates to put under the shelving. We went over the pest reports and what is documented on the reports. Ecob lab has visted the store on 1/24/24 1/29/24 and today 2/1/24.
