PIC also instructed to provide a DBA change(doing business as) for the establishment. A fillable Health permit application will be included in this Email for the owner to fill out and email to ISDHEALTH@BOSTON.GOV with all necessary documentation. I spoke to the owner on the telephone. Owner was instructed to list all persons included in the corporation on the application.
