Inspector notes: Old rodent droppings were observed along the stairwell, behind the dry storage racks, and behind the donut shelving.

Inspector notes: The presence of rodents is not controlled as evidenced by an accumulation of rodent droppings throughout the facility. The presence of insects, rodents, and other pests must be controlled by routinely inspecting the premises for evidence of pests, using acceptable methods to control pests, eliminating harborage conditions, and routinely inspecting incoming shipments of food and supplies. Establishment to continue with 2x monthly pest control visits until rodent activity is resolved.

Inspector notes: The following was observed: 1) The hand-washing sink in the kitchen was blocked by a chair. A hand-washing sink shall be accessible at all times for employee use. 2) Gloves observed in the front hand washing sink near the donut shelves.

The full evaluation must include: frequency of treatment, types of bait, traps, initial kill numbers, structural findings, and technician assessment. Establishment must fulfill all of the recommendations by pest control. Full evaluation must be sent to inspector. Pest control to increase frequency until rodent activity is resolved. - Establishment to create a plan of action to follow alongside the findings from the evaluation. Plan of action to include: cleaning and sanitizing of all affected areas with rodent droppings. Disposing of any adulterated product daily. Sealing openings that may allow for pest entry.

Inspector notes: The door to the outside, located in the back next to the basement, is not tight-fitting. Outer openings of a food establishment must be protected against the entry of insects and rodents by tight-fitting doors.

Inspector notes: The non-food contact surfaces were observed: 1) The front shelving near the register was observed with food debris 2) The speed racks were observed with an accumulation of food debris 3) The table under the espresso machine was observed with an accumulation of food debris The non-food contact surfaces must be cleaned as often as necessary.

Inspector notes: The door to the outside, located in the back kitchen area, is not tight-fitting. There is a gap at the base of the door. Outer openings of a food establishment must be protected against the entry of insects and rodents by tight-fitting doors.

Inspector notes: The following equipment was observed to be in poor repair... - The bottom drawer(s) to the cold drawer prep unit was observed to not close tightly and in need of adjustment. - The faucet in the back room area was observed to be leaking during times of use. Equipment is to be maintained in good repair.
