Inspector notes: There is no water between 100 and 110 degrees F in the employee toilet room and the 1st and 2nd floor toilet rooms with motion sensored sinks. The maximum temperature probed was 56 degrees F in the middle toilet room, 58 degrees F in the employee toilet room, and 85 degrees F in one of the second floor toilet rooms. There was water available at or above 100 degrees F in the infant room, one of the two second floor toilet rooms, and the second floor food prep area. Have children and staff use the second floor toilet room that has water readily available at 100 degrees F. New Violation.

Inspector notes: Chemicals (cleaning solutions) were stored above (on top of the refrigerator) or on the same shelf with food, equipment, and/or single service articles in the second floor food prep area and the first floor prep area. New Violation.

Inspector notes: Observed a plastic container of reusable utensils in the food prep area. There is no 3 basin sink. Discontinue the use of reusable utensils. Have parents provide all meals for their children. New Violation.

Inspector notes: Equipment does not meet minimum design standards: -Non-commercial reach in refrigerator, chest freezer, small reach in freezer, and convection oven in use. New Violation.
