Inspector notes: Observed disclosure method missing on menu items requiring consumer advisory on Specialties menu, Ramen menu, Poke menu and Happy Hour menu. Menu items requiring disclosure on Specialties menu are "Truffled Salmon Tartar" (with soft egg and salmon), multiple specialty rolls with raw ingredients and "Chef's Selection" omakase-style serving raw sushi fish. Menu items requiring disclosure on Ramen menu are undercooked "Soft Egg" add-on item and "Super Tan Tan Ramen" (served with soft egg). Menu items requiring disclosure on Poke menu are "Soft Egg" add-on item, "Ahi Tuna", "Salmon" and "Yellowtail". Menu items requiring disclosure on Happy Hour menu are "Salmon Nigiri" and multiple specialty rolls made with raw protein ingredients. Also observed reminder statement missing disclosure asterisk on all menus. Inspector notes: Observed 0ppm of chlorine sanitizer in the bar. Operator primed drained, started a new batch and primed the machine. The chlorine sanitizer registered 50 to 100 ppm at time of inspection. Discussed with Person in Charge (PIC) the importance of proper sanitizer levels and proper monitoring of sanitizer levels
