Inspector notes: -- At mop sink, observed a hose connected to an Atmospheric Vacuum Breaker (AVB) with a water supply line directly feeding a chemical dispenser. Reviewed hazards with the person in charge: AVBs should never directly feed a chemical dispenser because chemical dispensers create continuous pressure, causing AVBs to fail and creating a high-health-hazard cross-connection. AVBs are designed for intermittent use, and they must have their outlet open to the atmosphere. The manager disconnected the hose at the time of inspection. Ensure employees are reminded to never leave a hose on with constant pressure

Each of the items are placed in the unit around 9:00 AM today and was found at 110*F around 11:20 AM during the inspection. PIC voluntarily discarded the items. Discussed reheating times/temperature with PIC (2 hours to reach 135*F or above for items initially opened or still in commercial packaging). Discussed reheating methods with PIC and possible use of microwave or other equipment that can reheat faster or more reliably. Please ensure proper reheating times/temperatures are met when reheating TCS food items
