Inspector notes: Observed oysters and clams in seafood display case with no shellstock tags with the product. Shellstock tags were found in record keeping book. Shellstock tags must be kept with shellstock until sold, dated with final sale date, and maintained in chronological order for 90 days onsite. Also observed no date when the last Shellstock was sold or served recorded on the tag in the notebook record keeping system of at least two shellstock tags at the time of inspection. After discussion with the person in charge, it has been determined that the root cause of the violation was training staff (a verification breakdown), which resulted in the violation. Inspector notes: Observed by the ice machine broken and missing wall tiles, preventing wall and floor from being smooth and easily cleanable, as well as creating harborage conditions for pests. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection
