Inspector notes: -- No Certified Food Protection Manager (CFPM) onsite at the time of inspection. A CFPM is required to be on the premises during all hours of operation

Inspector notes: AVB on secondary water line for chemical dispenser is leaking when subjected to pressure. AVBs are not designed to be subjected to constant pressure, and it may no longer be functioning correctly. AVB is not required at this water line as the chemical dispenser has it's own backflow prevention device inside. AVB should be repaired or more advisably removed from this section. If backflow redundancy is desired, a continuous pressure vacuum breaker would be suggested. Please also verify backflow device in chemical dispenser is ASSE 1055 with large airgap rather than the model with small vents which can become clogged. Cover will need to be removed to verify

Inspector notes: Employee handling dirty dishes (pans and bowls) then proceeded to handle clean trays out of the dish machine and place them into clean storage. PIC had the trays, and the others they contacted in clean storage, placed back into the dirty side of the ware wash area to be cleaned again. PIC instructed employee that hand wash is required when switching from handling of dirty to clean dishes. Employee continued to handle dirty dishes
