All TCS food in equipment was holding at 41*F. All equipment must be maintained in good repair at all times. Discussed with person-in-charge (PIC) frequent monitoring of internal temperature of food items. PIC states they are replacing the unit in the next week or two

Inspector notes: During the inspection pans of cooked onions and cooked onions with jalapenos in covered pans at 50 to 55*F were observed cooling in the walk-in refrigerator. Per manager the items were cooling for approximately 2 hours. The manager placed the uncovered the items so that proper cooling procedures are followed. Through discussion with manager and reviewing log, temperatures were taken, however they were likely not taken properly to obtain a proper reading, and the items were placed from a sheet pan to the covered containers to soon. Reviewed how to properly take temperatures to verify proper cooling has taken place
