Inspector notes: Observed live roaches by cove base tile near the 3 compartment sink. There was a hole in the tile and some other broken tiles throughout the kitchen. Discussed integrated pest management and reviewed establishment's pest control logs. The most recent pest control visits were 7/16, 7/05, and 6/6. Per PIC, pest control was conducted monthly and was recently switched to weekly as they are aware of the pest issue. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Correct prior to re-inspection
