Inspector notes: Observed the low-temp mechanical dish machine with a concentration of 0 ppm chlorine while in use after three attempts. Verified with person in charge's (PIC) test strips. The sanitizer bucket observed to have sanitizer in it. PIC will have employees use the 3-compartment sink to sanitize until it is able to reach concentration of 50-100ppm. Discussed checking the sanitizer concentration every morning and logging the concentration and necessary corrections before the dish machine is used in order to properly destroy pathogens on the dishes. Please follow up with chemical company to locate the prime switch. Dish machine achieved chlorine concentration of 50 -100 ppm before inspection was over

Inspector notes: Observed 50+ fruit flies on the walls and ceilings in the dish pit, prep area, on the cook-line, and in the dietary storage room across the hall. Observed what appeared to be feces and larva from the fruit flies along the grid of the ceiling tiles. According to PIC, pest control treatments are applied every month. PIC will contact the pest control company for additional treatments. Discussed pest harborage conditions such as unprotected outer openings (doors and windows), pooling water and soil/debris. PIC will contact the pest control company, remove/repair harborage conditions and dispose of the non-living flies in the dining and kitchen areas. Re-inspection 10 days

Inspector notes: Observed the low-temp mechanical dish machine with a concentration of 0 ppm chlorine while in use. Verified with person in charge's (PIC) test strips. According to dish employee, the machine was tested prior to use, which was forgotten to be documented on the log. The chlorine bucket was replaced; the chlorine remained at 0 ppm after several cycles were ran. The repair company was contacted during the inspection. PIC will have recently washed dishes rewashed, rinsed, and manually sanitized and will continue until the dish machine has a chlorine concentration of 50-100 ppm. Discussed the importance of proper sanitizer levels and proper monitoring of sanitizer levels
