Inspector notes: Escutcheons/covers missing from walls, spacing around plumbing, and/or damaged walls below prep sink near walk-in cooler, near 3 compartment sink, and near cart storage at far end of ware wash station. PIC stated work order is in place for damage at cart storage area

Inspector notes: -- The ambient temperature of reach-in refrigerator near patient line measured at 54.5°F using min/max thermometer. See violation above. At the time of inspection, all TCS items were discarded from the refrigerator. The PIC will place a work order to repair unit, and instructed food employees not to use equipment for cold holding TCS foods until repairs are completed. Non-TCS foods such as bread and tortillas can remain in unit while awaiting repairs. The establishment has sufficient refrigeration space to safely remain open. Cold holding equipment must have the capacity to maintain TCS foods at 41°F or below. Must repair cold holding equipment such that it maintains TCS foods at 41°F or below
