Inspector notes: Observed bagged ice and ice blocks for retail sale with incomplete labeling. All packaged food labels must include the following items: (1) the common name of the food, or absent a common name, an adequately descriptive identity statement; (2) If made from two or more ingredients, a list of ingredients and sub-ingredients in descending order of predominance by weight, including a declaration of artificial colors, artificial flavors and chemical preservatives, if contained in the food; (3) an accurate declaration of the net quantity of contents; (4) the name and place of business of the manufacturer, packer, or distributor; (5) the name of the food source for each major food allergen contained in the food unless the food source is already part of the common or usual name of the respective ingredient; (6) except as exempted in the Federal Food, Drug, and Cosmetic Act § 403(q)(3) -(5), nutrition labeling as specified in 21 CFR 101 -Food Labeling and 9 CFR 317 Subpart B Nutrition Labeling; and (7) for any salmonid fish containing canthaxanthin or astaxanthin as a color additive, the labeling of the bulk fish container, including a list of ingredients, displayed on the retail container or by other written means, such as a counter card, that discloses the use of canthaxanthin or astaxanthin

Inspector notes: In the employee restroom, observed a household toilet seat. Discussed with the manager providing a U-shaped sanitary toilet seat before the next routine inspection. The manager stated that a new toilet seat would be purchased and installed

Inspector notes: Observed cracked tiles in the back room across from the ice machine and in front of the soda fountain. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection

Inspector notes: Observed a spray can of Hot Shot ant spray behind the front counter. Person in charge (PIC) stated spray was used for ants outside the back door. All pest control for the establishment must be performed by a certified pest control specialist and chemicals for pest control must not be stored onsite. PIC removed pest control chemicals from premises at time of inspection

Inspector notes: Observed bags of ice for sale with no labels. All packaged food labels must include the following items: 1) Name of Product; 2) Processor Name; 3) Processor Address or Phone Number; 4) Ingredients; 5) Approximate Weight; 6) In English

Inspector notes: Asked person in charge (PIC) if they have written procedures or clean up kit for vomiting and diarrheal events. PIC stated they did not. Provided PIC a copy of Guidance for creating a vomit and diarrhea clean-up plan. Written procedures for clean-up of vomiting and diarrheal events is required onsite at all times

Inspector notes: Observed a hose connected to the mop sink in the backroom with a metal clamp constructed of unapproved materials. Had PIC remove the hose from the sink and advised hose connections must be attached properly and removed when not in use

Inspector notes: Observed a hole in the corner of walls in the back room with accumulation of dry wall or other building debris on the floors as well as peeling on the walls. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Physical facility must be maintained in good repair at all times. Repair physical facility prior to next routine inspection
