Inspector notes: During inspection, a QUAT mechanical warewasher was observed actively being used to sanitize equipment, with a QUAT sanitizer level below the manufacturer's instructions/levels. The person in charge verified that the chemical concentration was below the appropriate levels using the facility's test strips and submitted a work order at the time of the inspection for the machine. The facility will rely on alternative means of sanitizing equipment until the appropriate chemical concentration can be established and verified. All equipment was rewashed and sanitized at the time of the inspection. Please ensure warewashing is conducted with sanitizer at the appropriate chemical concentration

Inspector notes: During inspection, observed the plumbing system below the 3-compartment sink to be leaking onto the floor of the main warewashing room. In a second instance observed the plumbing system below the 3-compartment sink to be missing. A work order was submitted by the person in charge at the time of inspection to repair/install the plumbing system of both items specified. Please ensure to maintaining plumbing systems in good repair at all times

Inspector notes: During inspection observed four unlabeled chemical spray bottles located throughout the facility. The person in charge stated the items were QUAT sanitizer and upon request the chemical spray bottles were labeled. Please ensure to have all chemicals properly labeled and identified at all times
