Inspector notes: Observed evidence of pest activity. Observed multiple sticky traps in backroom storage area containing numerous pests including crickets, cockroaches, spiders, and a lizard. Observed dead crickets in walk in cooler and a cockroach in the men's restroom. Reviewed pest control invoices with person in charge, last visit being 1/11/2026. Scheduled return in 9 days with person in charge to observe progress with pest cleanup and control. Establishment must be maintained free of pests, and all pest control measures must be conducted by a licensed pest control specialist
