Food is unsafe, adulterated, or misrepresented (corrected on site)
Inspector notes: Observed dots ice cream in a chest style freezer that had previously thawed and re-froze. Instead of being loose dots of ice cream, each container was a hard disk. A sign was placed on the freezer (out of order) and ice cream was embargoed. All food must be in good condition, safe, and unadulterated. All adulterated food was discarded at time of inspection. See embargo form

Evidence of pests or pest control inadequate
Inspector notes: Observed rodent droppings throughout the backroom. Several dozen droppings found all along the walls, behind stock, and under the stairs. Also observed an unapproved snap trap for rodents. Discussed that these traps increase risk of contamination. Must have a certified pest control specialist come out and eliminate rodents. Establishment must be maintained free of pests, and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection
