Inspector notes: Observed between 10-15 live German cockroaches located along the ceiling of the storage compartments underneath the hot roller machines. Per staff, this has been an ongoing issue and pest control services visit every 2 weeks to perform treatments. Audited pest control records. Pest control services were at the establishment on 8/8/25 to conduct routine treatments for cockroaches and rodents. Per pest control notes, no cockroaches were observed on 8/8/25. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist

Inspector notes: Observed multiple live German cockroaches as well as multiple dead German cockroaches located in the storage compartments underneath the hot roller machines and underneath the storage compartments near the F'Real machine. At the time of inspection, audited the establishments pest control documentation. Pest services were just at the establishment 3 days prior to inspection. Per pest service documentation, no pests were observed and the establishment was treated with traps and insecticides. In addition, observed multiple fruit flies flying in the front corner of the store near the pest service's wall sconce equipment. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection
