Inspector notes: Observed multiple live German cockroaches as well as multiple dead German cockroaches located in the storage compartments underneath the hot roller machines and underneath the storage compartments near the F'Real machine. At the time of inspection, audited the establishments pest control documentation. Pest services were just at the establishment 3 days prior to inspection. Per pest service documentation, no pests were observed and the establishment was treated with traps and insecticides. In addition, observed multiple fruit flies flying in the front corner of the store near the pest service's wall sconce equipment. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection

Inspector notes: Observed significant accumulation of dirt, dust, and other food debris throughout the establishment. Notable areas include: 1. on countertops underneath the soda fountains and beverage refrigerators (e.g. redbull, monster) and hot food display cases 2. in the storage compartments underneath the soda fountains and beverage refrigerators 3. fountain drink lid dispensers 4. all floor sinks throughout the establishment 5. men's restroom (counter top & plumbing underneath hand sinks). Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection

Inspector notes: At the time of inspection, the establishment did not have a Certified Food Protection Manager on site. Per assistant manager, they have not begun working to obtain theirs yet. A Certified Food Protection Manager (CFPM) is required to be on the premises during all hours of operation

Inspector notes: Observed 2 live German cockroaches located in the storage compartment underneath the hand sink near the coffee machines. In addition, observed another dead cockroach in the storage compartment near the microwave. At the time of inspection, the person in charge could not provide recent pest control invoices or reports. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection

Notable areas include underneath the soda fountains, underneath the slushie machines, the room behind the soda fountains, and in the compartments underneath the drink equipment and hot roller station. Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility to prevent the attraction and harborage of pests
