Inspector notes: Observed label on a bag of ice stored within the ice machine with labels that are missing the name of the facility the bags were made in, the address or phone number of the facility and list of ingredients. Discussed with Person in Charge (PIC) Labeling shall contain 1) Name of Product; 2) Processor Name; 3) Processor Address or Phone Number; 4) Ingredients; 5) Approximate Weight; 6) In English. Also discussed with PIC the bagged ice shall not be stored within the ice machine as cold holding unit and shall be stored with proper labels in a separate unit. PIC removed the bag of ice and will clean the machine before making more bags of ice

Inspector notes: Observed the water filter to the left of the three compartment sink to have a small leak and also observed the handwash sink in the back of the facility to have a small leak. Discussed with PIC to maintain all plumbing in good repair at all times
