Plumbing system not approved or fixtures not cleanable (corrected on site)
Inspector notes: Observed a spray hose with a spray head pressurized with water located at the mop at time of inspection. The spray head and hose are connected downstream from the Atmospheric Vacuum Breaker (AVB). Discussed that, unless disconnected after every use, no downstream shutoffs are to be installed downstream from the AVB. Discussed with the PIC that AVBs are not designed to withstand constant pressure and may fail to prevent backflow in as little as 12 hours. Advised PIC to disconnect spray head while not in use and reconnect as needed when actively in use. The PIC disconnected the hose and stored the spray head above the flood line of the sink at the time of inspection. Shut-offs, including chemical dispensers and sprayer nozzles, are not approved for continuous installation downstream from atmospheric vacuum breakers. PIC removed shut-off downstream from atmospheric vacuum breaker at time of inspection

Prohibited toxic material present in facility (corrected on site)
Inspector notes: Observed two to-go bags full with new, unused HotShot fogger pesticide, totaling about 20 individual foggers in the main office at time of inspection. Discussed proper pest control practices with PIC. All pest control for the establishment must be performed by a certified pest control specialist and chemicals for pest control must not be stored onsite. PIC removed pest control chemicals from premises at time of inspection
