Salad mix, tomatoes and ind salad mixes use time as control. COS: Staff created new mixture of salad mix and added proper time stamp.

Inspector notes: Observed nozzles at main drink machine in front house, milk shake mixing blade, and equipment plating, and bulk ice machine around circular chute at glue seal (mold was observed). Also dishes lg chicken containers were observed sitting dirty on the drying location for dishes. CFSM placed them in the 3 compartment sink to be re washed.
