Inspector notes: Observed facility with no health reporting form on file for current breakfast attendant. Forms observed were dated for 2020 and PIC stated that the current breakfast attendant had just started. Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. EHS informed the PIC of the need for anyone working in the kitchen to read and sign the health reporting document stating their awareness of the need to report any of the conditions noted.
