Inspector notes: Observed bulk ice machines with black substance on deflector plates. In equipment such as ice bins and beverage dispensing nozzles and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment: (I) At a frequency specified by the manufacturer; or (II) Absent manufacturer specifications, at a frequency necessary to preclude accumulation of soil or mold. Increase cleaning frequency of ice machines. Machine should be sanitized before being put back into use.

Inspector notes: Observed broken pipe from 3-compartment sink in pots and pan area draining directly onto the floor. Observed floor tiles broken, missing throughout the facility. Some areas holding water. All areas should be maintained in good condition. This has been an ongoing issue for years. Some of the tiles have been removed in large areas. Continue efforts toward compliance.
