Inspector notes: Cold TCS foods are not maintained at 41 degrees F or below. Observed cooked macaroni and cheese and a bag of cooked sweet potatoes being donated left on the preparation table unattended. The macaroni and cheese was observed at 57 degrees and sweet potatoes were 52 degrees. According to the PIC, both TCS foods were cooled at 10:30 pm on 4/21/2026. The items were observed on the preparation table at 10:00 am on 4/22/2026. PIC was instructed to discard due to time and temperature abuse.

Inspector notes: Observed the dishmachines final rinse concentration exceeded the manufactures recommendations of 50-100 ppm for Chlorine sanitizer. Cups were observed with a sanitizer concentration above 200 ppm. A pan was tested after observing the initial concentration of the cups. The chlorine concentration of the pan met the manufactures recommended concentration of 50-100. It was observed at 100 ppm. Due to inconsistant concentration levels, the PIC was advised to contact Autochlor and manually set up sanitizer in the three compartment sink until the calibration is corrected on the dishmachine. Also observed Quaternary Ammonium sanitizer exceeded the manufactures recommendation of 150-400 ppm in the three compartment sink. The sanitizer concentration was observed at 800 ppm. Inspector notes: Observed open employee cups, coffee cups, dining cups, and a bottled protein shake stored in the employee drink holder in the kitchen area. The location is using drink holder magnets to store drinks. Employees are allowed to store single use cups with a top and a straw in a designated area away from equipment, utensils, food, or food contact surfaces. PIC corrected on site by removing beverages that were not in single use cups and without lids and straws.

Inspector notes: Observed cooked okra stored below 135 degrees f below the pass thru heat lamp. PIC advised that the okra is reheated prior to service. The Operations Manager (Caleb) was advised that TPHC must be used and documented if the product is not being hot held at 135 degrees or above.

Inspector notes: Observed TCS foods in the cooling process did not cool to 70 degrees F in the required 2 hours. Chicken Fillet- confirmation of proper temperature at 11:34 am; the filet chicken was observed at 82 degrees at 3:35 pm. The pan of filet chicken was discarded. The Operations Manager (Caleb) advised that he is following the proper procedures. The cooling process is not monitored after the initial cool down process begins. The fillets are cooled in a slightly covered container for one hour and then loosely wrapped and placed on a sheet pan in the walk in cooler with the initial time and a 2 day discard date. Caleb was advised to monitor the temperature until the TCS foods being cooled reach 135-70 within 2 hours and 70-41 within 4 hours for a total of 6 hours.

Inspector notes: In use utensil (Spoon) used for sweet potato soufflé was improperly stored on the shelf above the hot holding area. Shelves above the utensil storage area contained old food build up. There was also a bowl being used to scoop cooked okra. A scoop with a handle should be used to prevent contamination. Utensil used to scoop pickles was improperly stored with safety equipment. PIC will relocate scoops and spoons to a clean, dry, location.
