Inspector notes: Observed live roach in cabinet under cash register. CA: The presence of insects shall be controlled to minimize their presence on the premises by 1. Routinely inspecting incoming shipments of food and supplies; 2. Routinely inspecting the premises for evidence of pests; 3. Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections and 4. Eliminating harborage conditions.

Inspector notes: Observed facility unable to provide a written Employee Health policy for food service employees to report information about their health and activities as they relate to diseases that are transmissible through food. CA: Facility must have written Employee Health Policy of procedure for employees to report to CFSM and/or person in charge to reduce the risk of foodborne disease transmission, including providing necessary additional information, such as the date of onset of symptoms and an illness, or of a diagnosis without symptoms, if the food employee or conditional employee. C Manager has 10 to email the employee health policy to the Dept of health EHS division

Inspector notes: Manager was not present and did not designate a person in charge. No one was able to answer food safety questions. CA (c) Correct Answers to Food Safety Questions. Responding correctly to the inspector's questions as they relate to the specific food operation. It is the manager's responsibility that someone is trained to answer EHS questions regarding food-borne illness and transmission of pathogens in the event she of indisposed.

Inspector notes: Observed no person in charge and the manager left no one in charge. CA 511-6-1.03(2) - PIC present (Pf) There must be a person in charge on the premises of the food service establishment at all times. Inspector informed that the manager needs to appoint a designated person in charge in the event she/he is absent

Inspector notes: Observed no supplies regarding clean up, employee was not able to help. C/A Must implement policy according to plan and ensure all supplies are kept on site to. Manager has 10 days to provide information and the supplies necessary.

Inspector notes: Observed no Quat test strips available to check the concentration of the sanitizer from the 3 compartment sink and sanitizer on the counter. C/A Must obtain Quat test strips to verify the concentration of the sanitizer

Inspector notes: Observed Quat sanitizer from the counter measuring at 100 ppm. CA: Ensure that Quat sanitizer solution is maintained at a range of 200 ppm to 400 ppm. COS: Quat sanitizer solution replenished to 400 ppm.
