Inspector notes: PIC could not demonstrate in a verifiable manner that employees had been trained on employee health and symptom reporting to the pic. CA: Discussed documents that verify employees have been trained on symptom and sickness reporting with pic. COS: pic will provide verification of all documents for employee training on employee symptom reporting and employee health policy for sickness to report to the Health Dept. Health Dept. will re-check

Inspector notes: Observed jar of spoons on counter in front dining room area not inverted to prevent hand contamination of mouthpiece. CA: Discussed inverting spoons during storage with pic. COS: pic washed and sanitized spoons and stored properly inverted.

Inspector notes: Observed clutter, etc. stored on floor in dry storage building where floor cannot be maintained clean. CA: Discussed having all items 6" off floor for floor maintenance with pic. COS: pic will put up new shelving and move all items off floor for cleanability. Health Dept. will re-check.

Inspector notes: Observed opened container ready-to-eat potentially hazardous TCS food (sliced bologna - opened 3/6, cheddar cheese - opened 3/8) stored in the facility exceeding 7-day limit. Corrective Action: Discussed date-marking procedures with PIC. PIC discarded the bologna and cheese.

Inspector notes: Observed warewashing machine with chlorine sanitizer not testing at minimum concentration with drying agent. The machine was not in use at the time, only the 3-compartment sink. Per PIC, there was a technician servicing machine the last week. The machine was installed after food permit was issued. Corrective Action: Discussed monitoring warewashing machine to ensure sanitizer on the utensil service measures 50-100 ppm at end of the cycle. PIC placed a "do not use" sign on the machine and will have it serviced by a licensed technician and all agents, including rinsing and drying, must be reviewed by the Health Department. Documentation of services and agents are required. The machine must be rechecked by the Health Department prior to use.

Inspector notes: Observed canned and jarred food items stored on floor of dry storage shed behind facility. Single use items were also stored on floor of shed. Corrective Action: Discussed storing food items and single-use items a minimum of 6" off ground with PIC. PIC will designate employee to relocate items until additional storage is added to shed.
