Inspector notes: Observed walk-in cooler condenser to have a leak. RCA: PIC shall have equipment repaired as to ensure safe temperature zone and avoid any possible contamination. All equipment shall remain in good repair.

Inspector notes: Observed a heavy build-up of frost/ice on the inner walls and bottom surface of front service salad prep top. Observed black build-up in well of steam well. COS: PIC removed food in cooler and cleaned the build-up of ice on walls and bottom surface of prep top. PIC raised temperature of prep top as to prevent any accumulation of food-items. PIC removed steam well container and washed, rinsed and sanitized before replacing. CA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
