Inspector notes: The PIC could not provide documentation or otherwise satisfactorily demonstrate during the inspection, that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food. CA: Ensure all employees preparing smoothies are informed of their responsibility to report illness. All employees that have food handling responsibilities shall have a signed form available for review during inspection. COS: Employee Health reporting agreements provided/copies made and onsite staff filled out forms.

Inspector notes: Vomit/Diarrhea clean up kit lacked EPA approved disinfectant. Facilities are required to have a written vomit/diarrhea clean up procedure that includes use of an EPA approved disinfectant with all supplies listed in policy on-hand

Inspector notes: Observed mold inside ice machine and build up of dust in lid grooves. Ice machine shall be cleaned at a frequency to prevent build up of mold. Clean ice machine within 72hrs.

Inspector notes: Observed front desk staff who prepare smoothies without hairnets/hats. No hats or hairnets available. Provide all staff with food responsibilities with hats/hairnets to be worn at all times when working with food.
