Inspector notes: Inspector began washing hands at the hand sink near the grill line and observed hand sink near grill line missing piping (to drain contents of sink to floor drain). Water drained directly onto floor. Corrective Action: A handwashing sink shall be maintained so that it is accessible at all times for employee use. PIC will have hand sink maintained within 48hours. Local Health Authority will verify the sink has been maintained.

Inspector notes: Inspector observed 5 of 7 light bulbs in the exhaust hoods not working. Inspector observed broken cutting board, steamer with broken leg, shelving with broken wheel, walk in cooler with flooring loosely attached to the unit. Corrective Action: Equipment shall be maintained in a state of repair.

Inspector observed pieces of carpet missing in dining room. Inspector observed front countertop with a broken corner.
