Inspector notes: Observed sanitizer solution in 3-compartment sink and sanitizer buckets with a ppm of nearly zero. COS: Employee discarded solution and remade solutions. Observed ice machine with "brown substances" on inside. Observed the ceiling of the microwave to be covered in debris. Observed the cutting Board being contaminated by employee items, a pencil and note pad was placed on top of it and later removed by PIC. Observed cookie cutters with old food build up on them.

There was also an open hole in ceiling of chemical storage closet. Observed dirty floors underneath shelves and equipment.
